Question of the Day
A new orthopaedic question every day at midnight, written in the style of FCPS-II, IMM and hospital induction papers. Pick an answer to see the explanation.
What is the optimal position of immobilisation of the hand to prevent stiffness (safe position)?
Answer: B. The safe (Edinburgh, intrinsic plus) position is wrist extended about 30 degrees, MCP joints flexed 70-90 degrees and IP joints fully extended, with the thumb abducted. In this position the collateral ligaments of the MCP joints and the volar plates of the IP joints are at their maximum length, preventing contracture.

What is the commonest site of rotator cuff tear?
Answer: B. The supraspinatus tendon is the commonest site of rotator cuff tears, usually about 1 cm from its insertion on the greater tuberosity in the hypovascular critical zone. Tears often extend posteriorly into infraspinatus. Ultrasound and MRI are both accurate for diagnosis.
